Who is eligible?
Any cystic fibrosis patient or Shwachman-Diamond
Syndrome patient in the U.S. or Puerto Rico under
two years of age.
With a prescription for ULTRASE®, ULTRASE® MT or VIOKASE®, the patient will receive:
- Free ULTRASE Enteric-Coated Microspheres, ULTRASE® MT Enteric-Coated Minitablets or VIOKASE®
- Free fat-soluble vitamins (pediatric drops)
- Free copy of Cystic Fibrosis: A Guide for Patient and Family educational book
- Free diaper bag with CareFirst for CFSM logo
How does CareFirst for CFSM work?
Once enrolled in CareFirst for CFSM,, the patient
then presents the AXCAN® Rx CompleteSM card
to a pharmacy with a prescription to receive free
ULTRASE® / ULTRASE® MT or VIOKASE®. Free
supplemental products will be mailed to the patient.
